Healthy Woman Physio respects your privacy and is dedicated to safeguarding your personal information. This privacy notice explains how we process and store client data. Healthy Woman Physio is registered with the Information Commissioner’s Office under registration number CSN8356219. All data is managed in compliance with the General Data Protection Regulation (GDPR).

The following information may be collected:

Identity data: name, title, date of birth, address, GP details

Contact data: billing address, email address, telephone numbers

Financial data: bank account and payment card details

Transaction data: details about payments to and from the client

Technical data: internet protocol (IP) address, login data, browser type and version, time setting and location, browser plug-in types and versions, operating system and platform and other technology on the devices the client use to access this website

Usage data: includes information about how the client uses the website

Special Categories of Personal Data: information about your current problem, past medical history, social history and any medications the client may be taking

Personal information is collected when the client gives Healthy Woman Physio their identity, contact and financial data by filling in forms or by corresponding or booking with the clinic by post, phone, email or face to face. Also, when the client and the physiotherapist complete the initial assessment form and when the physio records information during the course of the examination and treatment 

As the client interacts with the website, technical data may be collected about the client's equipment, browsing actions and patterns. This data is collected by using cookies, server logs and other similar technologies. 

Healthy Woman Physio website uses cookies. Cookies collect statistical data about the client's browsing actions and patterns and do not identify the client as an individual. It is possible to switch off cookies by setting the browser preferences.

Personal information is also used in order to process client bookings, to seek clients' views on the services provided, to notify the client of changes to services and send clients information that may be relevant to their treatment. 

If the client is unsure as to why a particular piece of information is relevant to their treatment, they must ask. 

Healthy Woman Physio will not share personal information without the clients' consent. Healthy Woman Physio will not sell or rent clients' information to third parties and will not share clients' information with third parties for marketing purposes. Healthy Woman Physio will not contact clients for marketing purposes by email, phone, text message or post. 

When clients give Healthy Woman Physio personal information, Healthy Woman Physio takes steps to ensure that it’s treated securely. Any information transmitted over the internet can never be guaranteed to be 100% secure. Healthy Woman Physio cannot guarantee the security of any information clients transmit to Healthy Woman Physio, and clients do so at their own risk. Once a client's information is received, Healthy Woman Physio makes its best effort to ensure its' security on their systems.

Under the UK General Data Protection Regulation (UK GDPR), Healthy Woman Physio is required to hold information to fulfil its legal obligations. By law Healthy Woman Physio has to keep information about its' clients in order to maintain accurate physiotherapy records. For clients over the age of 18, Healthy Woman Physio has to legally keep the clients information for eight years. 

All the personal information is stored in a locked filing cabinet within the clinic room which is locked when not in use. 

Under data protection law, client rights include:

Right of access -the right to ask Healthy Woman Physio for copies of their personal information. 

Right to rectification - the right to ask Healthy Woman Physio to rectify personal information they think is inaccurate. 

Right to erasure - the right to ask Healthy Woman Physio to erase their personal information in certain circumstances. 

Right to restriction and objection of processing - the right to ask Healthy Woman Physio to restrict the processing of their personal information in certain circumstances. 

Right to data portability - the right to ask Healthy Woman Physio to transfer personal information given to them to another organisation, or to the client, in certain circumstances.

The client is not required to pay any charge for exercising their rights. If the client makes a request, Healthy Woman Physio has one month to respond.

Healthy Woman Physio website contains links to other websites run by other organisations. This privacy notice applies only to Healthy Woman Physio, so the client is encouraged to read the privacy notices on other websites visited. Healthy Woman Physio cannot be responsible for the privacy policies and practices of other sites even if the client accesses them using links from this website. 

If a client links to Healthy Woman Physio website from a third-party site, Healthy Woman Physio cannot be responsible for the privacy policies and practices of the owners and operators of that third party site and recommends the client checks the policy of that third party site.
